Physical Night for the 2024-2025 school year is Wednesday, May 15th 2024 from 5:30pm-7:30pm. Please click here for more information.

Physical forms are required and can be picked up in the CHS Main Office, Athletics Office, or can be printed from the link below.

**Completed physical forms must be turned in to the Athletics office before any athlete is eligible to practice.**
